Background
this is not advertising, but more of like sharing my experience with the planning community (being a programmer myself)
on how to deal with issues that may arise in your project, which i am sure would be many issues that contributed to your project delays and lateness etc...
Solution
What we done, was we use iPAD to capture issues that are on the project, either it be building an aeroplane, building, trains
and by doing this, we have shorten the time it takes to get the issues from the identification into the systems which will alert management and repair team to fixed.
with the assist of the iPAD 2 , we can also be able to take photos, so the repair team can know exactly what the problem was.
it was a time saver and i am sure many programmer here will benefit of this technology.
Demostration
we actually have developed the application and commercialising it. to take advantage of tablets (iPAD or Android)
in capturing the issue right there and get the data back to the repair team without writing it on a piece of paper and then re-typing it in the computer later.

I have never yet had to produce a specific bar chart for defects liability - the erratic and unforeseen nature of the work makes it impossible.
It is usually done with a puinch list which is then sorted into some sort of priority order - on a spreadsheet.
When the defect is repaired the item is ticked off the sheet.
The example that you have stated has no relevance to construction programming.
