I have multi resources assigned to an activity.
Example: scaffold - 3
equip. oper - 1
forklift - 1
Now I have 5 scaffold - 0 equip. oper and 0 forklift.
This has happened on a number of activities but not everything. In some cases only one resource has doubled.

Although I do not use P6 most software have some similarities, when you mentioned multi resources I interpret it to mean a functionality available in other software and perhaps in P6, under Spider Project it is not the same to assign multiple resources each with their own quantity or assigning a multi-rersource, similar in name but different functionalities available under Spider Project and perhaps under P6.
If the issue is not with multi-resources as described above then the only other functionality I know that behave as you described is when you use skills, perhaps by error when editing or re-naming some resources these resources were automatically assigned to be under the same skill. A possibility if this functionality works similar on both software.

As this was posted under P6 I assume that your question is related to P6.
For me this automatted modification is mysterious. I never had this experience.
First I would check project's integrity (open this project --> Tools --> Check Project Integrity --> Mark "Log to File", but NOT the automatted correction).
This will show possible inconsistencies within your project.
What are your activity settings under "General" - especially "Duration Type"?
Please tell me the result of your check or send me the log file.

Depending on your particular software is how multi-resources can be edited. The following is from Spider Project Help Page.
Participants contains the list of resources comprising the current multi-resource. For each resource its name, code and units quantity in the multi-resource are displayed.
Quantity – sets the number of resource units in the multi-resource.
Add – lets the user add new resources to the multi-resource. Pressing Add button invokes the dialog box with a list of project resources. The user can select one or several desired resources from the list with the left mouse button and press OK. Note that: a resource cannot be added twice to one multi-resource.
Delete – removes selected resource from the current multi-resource composition.
Update content – enables the user to update the current multi-resource assignments globally in the project with new multi-resource properties and composition. Note that: historical project data cannot be updated, i.e. in the ongoing project with included performance data updating of multi-resources properties will not affect data before the project data date. The current multi-resource activity assignments are updated with the following peculiarities:
Multi resources is one functionality that perhaps do not add value to the resource leveling per-se as you can do the same with multiple individual resource assignments. But this is a functionality so convenient for resource loading is the one I use to load my crews as they move through many activities, it would be insane individually load the same crew on individual resource assigments to every activity, not to mention when several crews work on the same activity with different workload or partial assignment.
We do not change crew composition every other week as the crew moves to do work in other activities, modeling optimal crew composition for resource loading of each activity in our model would be misleading. Yes multi resources can be edited at the activity level but be careful of moving in the opposite direction of how crew composition will be managed on the site.
