You have to clarify first, who has initiated for the design changes? is it from the employer/engineer or from your side. Since this is the lumpsum contract, so i really doubt on the slab item since you are doing some modifications, may be this can be a minor or major... will the area/volume of the slab changes dramatically?. no. because the bill will show only the slab volume for a particular grade of concrete, is it not?. or do you have any break up for steel quantities in slab in your contract BOQ?. these things need to be clarified first, also as per FIDIC 15% variation can be allowed in the total contract sum( i am not sure what has been mentioned in the conditions of particular application). you have to mention whether these changes affects the entire building or not? if it is the case you can ask for time extension or the consultant will advise you to mitigate the schedule. if you foresee any real impact on your originally planned sequence of work, you can go for time extension. may be you might have purchased formwork to take care of the previous design condition, is there any provision to tackle this situation, may be you can find it in your contract.
If you are asked to carry out additional works (any new scope) then i think we can easily go for claiming the money.
Ofcourse this is a contractual matter, you can claim for money and as well as for time.
