There is agreat package in the UK called "Risk Aid"

There is an excellent tool available through Primavera called PERTMASTER RISK.
Pertmaster used to be a stand alone projectye management package, they then developed risk management into the software.
You can still use it to simply plan and monitor your project but have the option to analise risk as well.
I understand that the top version can also analise financial risks also,

The planner can help produce a risk regeister but the PM should be the person who ensures that it is done to the required level of detail.
A risk register can be just be an excel spreadsheet that records risk names, probabilities, impacts, mitigations and owners.
If asked to model this risk into a schedule, the planner can use tools such as @trisk or Pertmaster to impact the schedule and cost with different risks, levels of uncertainty etc.
