Time constraints must be set on the topics in the agenda.
If the time constraints are overrun, a seperate meeting must be arranged with only the people concerned.
The chairman must be in control at all times, and take action on diversions when they occur.
The biggest problem with meetings is the number of people that is involved, so try keeping certain topics such as safety at the start of the meeting, so that the concerned parties can leave the meeting as soon as posible.
Minutes are very important, and you must ensure that there is somebody proficient taking the minutes. The minutes must be typed and circulated ASAP, preferably within hours, so that parties who have been actioned can perform their actions ASAP, and have no right to say they did not know.

Try getting hold of a "Toastmasters" manual.
The main points is:
A meeting must have an agenda.
