EPDS (Engineering Project Data Sheet)

Hello I am trying to create a template for our group of a EPDS-Engineering Project Data Sheet. It needs to have easy fill in the blank capabilities and also it needs to have 2 or 3 layers.

Does anyone have or know of a template of something like this?
